What to bring (insurance forms):

• Commercial insurance claim form and insurance card, OR Worker's compensation employer information, OR No-fault automobile insurance information.
• Driver's License
• Please bring referral form

What to wear:

• Please bring comfortable clothing and sneakers.
• You may bring shorts or use ours if we will be treating your leg(s).
• On your first visit, please arrive 15 minutes before your scheduled appointment to complete the required paperwork.
• Your first session will last approximately 1 1/2 hours.
